False High Troponin Level Originated From Gelatinous Fibrin in Serum: Two Cases

Introduction: We aim to emphasize that importance of intralaboratory preanalytical control process with describing two cases of false positive troponin level originated from fibrin gel.

Cases Presentation: Troponin test performed on Beckman Coulter Access 2, CA, USA instrument with original reagents. First and after second centrifugation troponin level of first case was measured 0,0260 ng/mL and 0,0103 ng/mL respectively (reference range 0-0,0183 ng/mL). The patient has been under anticoagulant treatment and PT and INR result were measured 48,5 second and 2,74. First and second troponin measurement of second case was 0,1948 ng/mL and 0,0093 ng/mL respectively. Second case has been under anticoagulant treatment too and PT and INR result were 49,1 second and 2,76. It was observed that the test process was completed in one hour in both cases.

Discussion: Detecting false high results in troponin anlysis of these two cases show inappropriate coagulation, despite presence of clot activators in primary serum tube. Serum samples that are allowed to sit for 30 minutes prior to centrifugation can remain insufficient in patients under anticoagulant treatment. Specimens of patients receiving anticoagulants allowed to sit longer than 30 minutes are both will be benefit for patients and increase laboratory reliability. Additionally troponin assay which is reporting the rapid results are important in clinically can be consider to perform in plasma specimen according to hospital and laboratory conditions.
