Comparison of Primus Ultra2 Analyzer and Tosoh HLC-723 G8 HPLC Systems in Thalassemia Screening

Objective: HPLC (High Performance Liquid Chromatography) is the most commonly used method to differentiate the subtypes of hemoglobin in screening for hemoglobinopathy. Today, different models and trademarks of HPLC equipment have been used in screening for hemoglobinopathy. However, the agreement of the results of the analyses performed by these types of equipment are unknown. In this study, the aim was to investigate the agreement between the results of the analyses of hemoglobinopathy screening performed in two sets of HPLC equipment with the Primus Ultra2 analyzer and the analyzer Tosoh HLC 723 G8 trademarks.

Materials and Methods: A total of 40 individuals, 22 males and 18 females, presented to the laboratory between March 15 and April 15, 2013 for thalassemia screening were included in this prospective study. Statistical correlations of hemoglobin A2 (HbA2) values, analyzed in each analyzer as a single analysis from each sample were evaluated using student t test, two-way random intraclass correlation coefficient (ICC), concordance correlation coefficient, correlation coefficient (r) and regression analysis.

Results: As a result of the analysis, 6 of the samples (33.7%) were detected as -thalassemia carrier, 10 (36.2%) were diagnosed as anemic and 24 (30%) were detected to be normal. HbA2 values were not significantly different between the two HPLC equipments (t= -1.269 p= 0.208). Correlation coefficients for HbA2 values were found to be as r= 0.987, ICC=0.97 and CCC= 0.93. Regression analysis for HbA2 values were found to be as Y= -0.0605+1.1613X.

Conclusion: According to the regression, correlation and other statistics analysis, agreement between the results of HbA2 values measured in the two analyzers was found to be sufficient.
